Consumer Health Data Privacy Policy

Effective date: September 1, 2026

This policy explains how Anghel Facial Plastic Surgery (Anghel Plastic Surgery PLLC) collects, uses and shares “consumer health data,” as defined by the Washington My Health My Data Act. It applies to personal information that identifies, or could reasonably be linked to, you and your past, present or future physical or mental health.

It does not apply to protected health information that we handle as a health care provider under HIPAA, such as your consultation and treatment records. Our HIPAA Notice of Privacy Practices covers that information.

Consumer health data we collect, and why

Our website does not ask for health information, does not use analytics, and does not use advertising or social media tracking. We collect consumer health data only in these ways:

  • Contact form details: the procedure(s) you are interested in and the time frame you have in mind. We use them to call you back and schedule your consultation.
  • Information you choose to send us: if you include health details in a message even though we ask you not to, we use them only to respond to you.

We do not use consumer health data for any other purpose unless we first tell you and get your consent.

Where we get it

  • From you, when you use our online contact form, book online, email us or message us.

Consumer health data we share

None. We do not share consumer health data with third parties, and we have no affiliates. Our contact form provider (monday.com) and online booking platform store this data only to provide their services to us, under our instructions.

We do not sell consumer health data.

Your rights

If you are a Washington resident, or your data is collected in Washington, you can:

  • Confirm whether we collect, share or sell your consumer health data, and see that data.
  • Get a list of the third parties and affiliates we have shared it with, and how to contact them.
  • Withdraw your consent to our collecting or sharing it.
  • Ask us to delete it.
  • Appeal if we deny your request.

To make a request, email hello@anghelfps.com with the subject “Consumer health data request,” or call 360-450-4359. We will confirm your identity, then respond within 45 days. If we need more time, we will tell you; the law allows one 45-day extension when reasonably necessary. We will not charge you or treat you differently for using these rights.

If we deny your request, you can appeal by replying to our decision. We will answer your appeal within 45 days. If you disagree with the result, you can contact the Washington State Attorney General at www.atg.wa.gov/file-complaint.
